[sakai2-tcc] Revised individual Sakai CLA process

Interestingly, I went through this process for Sakai recently when it 
was discovered that I (somehow) had never signed an ICLA.  Anthony was 
still handling it at the time.  Fortunately, I am able to scan a signed 
document and mail it.
