What version are you seeing this issue?  Did you test this on trunk?  [1]  Clear your browser's cache before testing and if it happens again, take a screen shot and create a jira.

I notice that on the “Tests & Quizzes” page in the “Create from Scratch” window, the actual “Create” button is rendered mostly out of the window, and the label (Create) was not visible in Firefox. However if I widened the overall browser window (it was already 80% of screen size) they this sub-window reflows to display it. Experimenting further, making the overall window narrower, the display of the sub-windows gets further broken…
 In Chrome & IE it is fine.
