[sakai-pmc] Published Sakai Upgrade Survey results 2014

Hi PMC,

I've published the Sakai Upgrade Survey results [1].  I'll announce to 
the community early next week.

Two questions:

1) Should I list the institutions which participated in the survey, as 
part of the summary?

2) Should we put out the Tools survey again, with survey component and 
SQL component, like we did last year? [2] ?
