[Building Sakai] LTI launch to URLs on ports other than 80

Are any load balancers involved? If a load balancer is forwarding the LTI
launch and changing the port in the process, the OAuth validation will blow
up. The phenomenon is described in
https://jira.sakaiproject.org/browse/BLTI-197.
