[Portfolio] Evaluation and Group workflow

Groups should solve this. Evaluators will only see requests from people who are in a group that they are in.

We have two modifications which we'd be happy to see adopted:

1) Sometimes you want a site owner who can see everything. We have an additional "see all" permission so that some roles bypass the group check.

2) We'd like to capture data for reports showing what group a given evaluation was for. (We have a professional school with several programs in it. We need to report portfolio activity for each program.) Thus we added a table that gets an entry when the student submits for evaluation. It records the page submitted and the group that student is in. If they're in more than one they're asked to specify which applies. We record the group title, since over time groups in a site may change. We need to be able to show the group even if it's no longer in the site.
