[Portfolio] question about default names for saved participant and item-specific feedback forms

Lynn,

I was just in a workshop today where I had to explain why the default value
for that form name is not a good choice, especially the way we use forms
from matrices in portfolio templating.  It would certainly help if the
default name were shortened enough to even read it in the Form Name box.

I¹d vote +1 from VT.

As a side/related note, was that IUPUI or somewhere else that had customized
that field so that it was by default blank and the user had to enter a name
to proceed?
