[Contrib: Evaluation System] compact and full item rendering

Thanks again for the feedback on the multiple choice, multiple answer, and
block item renderings.

Here is a proposal for compact and full scale item renderings. As before:

1. Corrected rendering in browsers that did not exist when the items were
first designed.

2. Added hints to help user answer easily, know what items and how they have
been answered.

3. Added A11y and keyboard navigation enhancements.
