[sakai2-tcc] Revised individual Sakai CLA process

I agree, I would expect the process for 99% of people (who are actually
going to be using a computer to do svn commits anyway) nowadays to be:

- Download form
- Use software to fill out form
- Use camera/phone/scanner to capture signature
- Paste signature into form (Newer Adobe Reader even has a wizard that
walks you through this sign/photo/attach process)
- Email or upload to designated area
