[WG: Sakai QA] 2.6.1 update (refresh to 2.6.x -r67199; blocker remains)

I've updated the sakai-2.6.x-qa branch to 2.6.x, r67199. See below
for a list of Jira issues addressed since the last refresh (r67107 on
2 Oct). QA server admins supporting this release should deploy this
2.6.x-qa refresh at their earliest convenience.
One blocker stands in the way of cutting the 2.6.1 release: conversion
scripts. Matt Jones has updated the conversion scripts but they have
yet to be merged into 2.6.x, pending testing verification. Testing
out these scripts by someone other than Matt would be appreciated.
Note also that Matt's script work does not cover DB2.
Please review the details below, particularly the small set of non-
blocker open issues marked with a fix version of 2.6.1. I recommend
they be bumped to 2.6.2.

"I have run these scripts multiple times testing out the fix for
SAK-16548 and verifying the other conversions are in. I haven't found
any issues with it and have confirmed that it fixes the errors. It
would be appreciated to have someone else agree with this though
before it's signed off on."
DB2: also as Matt reported on 2 Oct 2009:

2.6.1 OPEN ISSUES (n=7)
Not counting the single blocker, there are six other issues with a fix
version of 2.6.1 and a priority of "major" or less that are not
resolved. I recommend that these 6 issues be bumped to 2.6.2 by
their respective assignees (Zhen, Megan).
