[sakai-pmc] Community upgrade survey

Last year, around February, we had a Community upgrade survey.

I think it would make sense to do the same survey, with the same 
questions, for this year. Slightly modified to account for the 
possibility that an institution might have plans for Sakai 10.

I'll draft something up in the next couple of weeks, or at the Apereo Camp.
