[Building Sakai] Unexpected login prompts - Sakai 2.6.0

We periodically have a problem where a user is in the system working (constant activity – not a timeout issue) and Sakai unexpectedly prompts for user name and password. A variation on this is that Sakai will prompt several times. The correct credentials are entered several times and then the mangled screen shown shown below appears. The log shows this:

2010-06-12 16:02:01,321  WARN http-443-Processor5 edu.amc.sakai.user.JLDAPDirectoryProvider - authenticateUser(): invalid credentials [userLogin = jsmith]

2010-06-12 16:02:21,649  INFO http-443-Processor13 org.sakaiproject.login.tool.LoginTool - complete: nowhere set to go, going to portal

Any suggestions about what might be going on?
