[Building Sakai] The tools - Forms

Best way to start may be to use the "form builder" contrib tool to
generate some xml schema files that you can use to create a form.

You might try importing a sample "matrix" like this one here:
http://openedpractices.org/resource/aacu-essential-learning-outcomes-matrix-and-value-rubrics

Which will also import about 16 forms for you to see, play with and
tweak. It also will give you an idea about what it might be used for.
