[Building Sakai] Test & Quizzes Nuance - score in grade book but no submission attempt listed

Any ideas, suggestions...
 
We have an instructor who sees a grade in the grade book for a student but when they try to view the submission for that specific test under tests and quizzes it reflects that the test was never submitted by that student.
 
We are trying to figure out how/why this would have happened.
