[Building Sakai] Samigo: Retake assessment problem

Has anyone had a problem with students not having the option to retake
an assessment even though it appears they should be allowed to?

It looks to me like there could be an incorrect recording of the number
of times the assessment has been taken, but I haven't yet been able to
track down the exact problem.
