[sakai-core-team] Merge questions with Samigo question pool fixes

Hey Matt,

My two tickets (SAM-2385 and SAM-2395) are independent from Raul's 
(SAM-2274, SAM-2368).

I marked them as related because in order to test my fixes, Raul's also 
needed to be applied (to fix question pools in general).

I think if you did Raul's first, and then mine, it'd be all good. Hope 
this helps. If you have any questions feel free to ping me.
