[Portfolio] Adding a form to a published portfolio

Does anyone know of a way (clunky workarounds are perfectly acceptable) to
add a form to a cell in a published portfolio?

We have a portfolio for a 5-year program that's been in use for 2 years.
The first cohort just got to a few cells for the 3rd year that are missing
forms.  *sigh*  Help!
