[Contrib: Evaluation System] Patch for instructors to add their own items?

Hi Joseph -

We developed the patch for Columbia and they are using in their production.
I think there were a few more patches to the functionality after that
initial commit - I can check and update the patch if you're interested.

The idea was to allow instructors to add questions in the "preview" view -
wasn't sure if this was an approach that would have broad support in the
community so I held back on committing it to trunk. But if folks feel its
worth it, I can commit it to trunk.
