[Contrib: Evaluation System] Some UI updates to run by the community

Hi all,
UC Berkeley is running a pilot with Eval Sys this semester. In prep
for the pilot we've been doing requirements analysis for our campus
along with user experience and gap analysis of the existing system.
For Spring we are going to make some fairly minor updates to the
UI...mostly visual and information design...to support students
taking the evaluations and instructors viewing results. Aaron and
other at Unicon are doing the development work with us. As we move
forward to future phases, we'll likely get into some of our deeper
requirements around evaluation management including some visual and
information design to the administration pages. For now, I wanted to
share our Spring updates with the community and find out if they
should be more broadly adopted, which would be out best case
scenario. I've tried to make minimal changes and keep mind that many
of you have been running this system for some time now. That said,
I'm new to the community so could be missing something that is more
subtle. And of course, it is different for those of you who have
users already familiar with the system as it is.
Being new to the community, I'm also not sure what your process is for
getting buy in to make these changes in trunk so I could use some
advice there. I'll start with sharing the designs and jiras and
starting a conversation around them. Feedback, questions, general
thoughts encourages and welcome!
