Hi Beth,

This sounds like https://jira.sakaiproject.org/browse/EVALSYS-1179 . 
There's a syntax error in the default e-mail template for the new 
'submission confirmation' e-mail. Eval response submission fails 
silently if that template is used I submitted a fix which was merged to 
trunk (r80434), but I don't think it was merged to 1.4.x. To verify 
that's the issue, turn off the sending of submission confirmations, or 
make the change described in this patch to the e-mail template through 
the UI:

If you can submit a response after taking either of those steps, that's 
probably it.
