[Contrib: Evaluation System] confsuing template behaviour

In addition: Q1 from T1 will be in the question bank but the copied then edited Q1 on T3 is not in the question bank.

Therefore the question bank does not contain all the questions that a user has used, only the ones that are created via the 'Add' button and not by virtue of being copied!

In summary, questions that appear on a template that is a copy do not appear in the Question Bank and there appears to be no way of adding them.
