[Contrib: Evaluation System] QA5 Server Updates

Thanks for your patience! qa5-us.sakairpoject.org us up and running on the
copied Oracle 11g database instance now. It is still running the previous
tag of evaluations (1.3.0-RC4), and this will be updated to the final 1.3.0
*soon* as mentioned by Beth.
