[Contrib: Evaluation System] provider not being called

It appears taht EVAL_ASSIGN_USER is properly loaded. Looking at the DB query issued by the summary page, it looks like the problem is that our evaluations were set up with a state of Viewable, and they're now supposed to be Active.

The student summary now works. I can't get "My Surveys" to return. It's spending all its time in hibernate in EvalEvaluationServiceImpl.java. It's thrashing the app server CPU, not the database. Here's a typical backtrace:
