[Contrib: Evaluation System] 404 Error on Add a comment link

I think it was 1.2.1 but when I checked the contrib source the versions
didn't match up. So I may have grabbed a trunk version (yes, shame on me)
after 1.2.1 because of something I wanted at the time.
