[Contrib: Evaluation System] 404 Error on Add a comment link

Thanks for steering me in the right direction. I was able to fix it.

The 404 would occur in Firefox and IE. It did not occur in Chrome. The
problem was that I had an older version of toggleCommentBox.js where
"console" was still referenced for debugging purposes. This was causing an
undefined error. I removed these references and that fixed it. I should
probably consider updating the tool sometime.
