[Contrib: Evaluation System] adding more scales - error

I was trying to add some 4pt scales to our version of the evaluation tool. I'm a super admin but when I click on "add new scales" I get 

"Sorry! Something went wrong with this tool. The Helpdesk has been automatically informed, but please feel free to email us to give us more details about what happened."

What am I doing wrong. does it matter that we havent set up the hierarchy?
