[Contrib: Evaluation System] Help with Evaluation tool Error Message

Josh,

Was the evaluation released to multiple groups, and if so are you in the 
Instructor/maintain role in each course? I also received this error if 
an eval was released to (for example) 3 courses and I was only the 
Instructor in 1 of the courses.
