We had a similar need and didn't find any way to match the survey to the student name. Sakai does keep track in the system and reminds participants to complete the survey and not allow them to submit more than once (if set up with those preferences).
 
Our workaround --we requested in the first question their Sakai ID, then First name, Last name.  We did this so when we exported the results we would have the necessary information needed post the spreadsheet back into Sakai using Post'em.  This allowed each individual student to see their selections confirmed.

Instead of one question with 15 choices and asking students to select 8 we created 8 questions with a possible 15 options (copying the same question). For questions 1-8 they can only make one selection and were told to make their selections in the order of their preference.  This kept any 'over' mistakes from happening since allowing multiple answers to one question requires students to count the number of selected buttons and occasionally they chose more. A nice feature that could be added to the eval tool is the ability to specify the number of responses in multiple choice questions- multiple answers equal to 8.  Our other survey application does have this capability and it's been very useful.  The only possible glitch to the above is a student who selects the same section twice - we made the directions as clear as possible--stating sections will be assigned for any selected duplications? Cutoffs for sections were assigned on order of preference and first come first serve basis. 

You didn't mention if there was a limited enrollment in the sections but if there is having an order of preference might help. You'll have to manipulate your data a bit - we worked our way across the spreadsheet grouping each column by section (expanding the selection to keep the data together) - created a second spread sheet and made 8 rows representing each section.  From the 1st spreadsheet we copied the last name of the students after grouping section numbers in each column.  We copied the names from the 1st row and paste it into the indicated section column on the 2nd spreadsheet, go to the next row, group, etc. -- so then you have a 2nd spreadsheet with each column title listed with a section heading number and the last name of the students enrolled

Another possible tool to complete this task is using the test & quizzes tool.

Is there a way to get Evaluations to record how a particular site participant has responded? 

We have an idea to use it for course sign-up - one question, multiple responses: "Please select the 8 courses you wish to attend next year". When everybody has responded the whole shebang can be exported to Excel and processed.

If this cant be done we could insist that people give their name in a compulsory text field.
