[samigo-team] SAM-2070: re-org of settings

This is a quick note that I have attached a patch to SAM-2070 to
consolidate the 10+ categories into 4:

I did not re-do functionality like changing radios to select buttons. I'm
happy to take a look at some of those easier requests once this large piece
of code is committed.  I'm happy to commit with confirmation from someone
at Stanford.
