[samigo-team] Samigo Issue: Multiple answers from the same Submission ID

I agree, it would be bad to not have the proper error handling,
especially if it destroys the whole session.  I guess that's worse
than a student getting X times their score on a question and being
unnoticed by the teacher (Sakai lottery!).  Just depends how bad it
fails (which we don't know).  Thanks for looking, we should at least
create a jira for this and keep track of it.  I don't really know
where we can go from here w/o being able to reproduce it and know
where it comes from.  Ideas?
