Thanks to Chuck H. 

I didn't realize that Student Comments can be Graded in Lessons. That is how one can reproduce on 2.9.1 or 2.9.2  .  It is already fixed in the nightly 2.9.x server, so you cannot test there! and it is fixed in Rutgers QA server. Maybe try Longsight - qa29.longsight.com ? (I used localhost).

1) Add a Comments area
2) Make the Comments area grade-able (edit it and you will see where you can assign points possible)
3) log in as student, put in a comment
4) grade it as instructor. Look in the Gradebook as instructor. It is there.  Check the "Course Grades" button. Grade is incorrectly inflated? 
5) Log in as student, you do not see the grade. 

* Seems like it calculates the grade incorrectly by just adding the points received for the comment but not the points possible, so it boosts the student grade?  I'm not sure if that is the calculation issue, but the first thing I'm seeing.
