[WG: Sakai QA] 2.6.1 update (refresh to 2.6.x r67394)

I have refreshed the sakai-2.6.x-qa branch to r67394.  The following  
new fixes are included:

SAK-17080 add 2.6.1 conversion scripts
SAK-16786 assignments
SAK-13771 search: change get transaction id failure from error to warn  
noting inapplicability to PRE-1.8 versions of HSQLDB.
SAK-15944 citations: revisions 59143, 59368, 60018, 60502.

I will now commence work on creating and prepping the 2.6.1 release  
branch.
