[sakai-pmc] Final (?) Draft of Sakai Community Survey

Hi,

I added a role question. I made up the "Executive Management" option. Not sure why a Director (separate choice) would not be Executive Management. The rest of the roles are verbatim provided by Concentra. I made slight wording modfications to a couple of other questions. 

I think I would like to do a test run of the survey on one or two select folks (other than PMC) to get feedback, before publishing broadly.
