[Building Sakai] assignment - students received the feedback with the different content he submitted

Hello all,

We have a student who received the feedback for an assignment, but the
problem is the content of the assignment with the feedback is different
from the content that he submitted.
I am guessing the instructor may open two browsers when he grades but I am
not sure.
My first question is there is anybody having the similar experience.
The second question is how to fix this issue such as retrieving the
original submission data.
