[Building Sakai] Student timed assessment answers not recording changes

Has anyone seen an issue like this one? Students report that when they take
a timed assessment (Samigo), they go back and change one of their answers,
but the first answer that they entered is the one that gets submitted.

I have been trying to replicate it with a variety of browsers, but no luck
so far. Any info would be appreciated.
