[Building Sakai] 'Save and Release to Student' does not always return assignment to student

Hello,

We have observed sporadic cases in Assignments where a faculty
comments on an assignment and presses 'Save and Release to Student',
it does not set status to Returned.  No error is seen but the
assignment is not returned to the student.  It does not happen for
other students in that class; sometimes you can later log in and
successfully 'Save and Release to Student' to the student that
originally would not return.

We are running rSmart Sakai 2.7.1.0

Has anyone else encountered a similar issue?
