Thanks for your comments, Michelle.  It is possible you may be on to something.  The VMs we setup were started with auto.ddl=true. It's possible that the folk running the production hardware started Sakai initially with auto.ddl=false.  At least, that's what it's set to now so I'll talk to them.

I'm looking at my VM instance and see that I can choose an existing assessment type and that option is not available on the production hardware.  That would seem to tell me that the template data are not there.

What is the db script you're referring to?  I'm new at this and am still finding my way around and cannot find this script.

This is probably totally unrelated, but we were seeing this error on our test environment when the samigo template data wasn't in the db on startup (we don't use auto.ddl and were evaluating the tool).  We had run the db script to add the template data after startup, so we were unable to create assessments and saw this gradebook message.  In our case, we just needed to restart tomcat to get everything initialized properly.  Have you checked that the template data is in the db?  Other than that, my samigo knowledge is very limited.

Tests & Quizzes -> Assessments -> Create gives the message 'Warning: Gradebook has not been deployed properly. ....'

Tests & Quizzes -> Assessments -> Quick Create gives us an error message starting with:
org.sakaiproject.portal.api.PortalHandlerException: org.sakaiproject.tool.api.ToolException: java.lang.NullPointerException
   at org.sakaiproject.portal.charon.handlers.ToolHandler.doPost(ToolHandler.java:70)

This operation works fine for us on VMs with a MySQL database but the errors occur when we switched over to the production hardware which is using an Oracle database.  No migration was involved.  The Oracle db was created during the initial startup of Sakai on this hardware.

I see mention of this problem in SAK-16281 but the resolution is Cannot Reproduce.

Can anyone give me any insight as to what needs to be done?  This is a show-stopper for us until it can be resolved.
