[Portfolio] Coordinator/Reviewer/Public not able to view Matrix Form attachments when used in Portfolio

Today i found that url's generated for attachments are wrong. Each attachment
is associated with different form but the generated URL's of all attachments
are containing the same formId(1st formId). Because of this reason
checkFormAccess always adding attachments of 1st form.

1st attachment URL:

Note: Form Id is highlighted in bold and italic format in the above links.

Also I printed the text of generated XML document generated by the
createDocument method of
org.theospi.portfolio.presentation.model.impl.PresentationManagerImpl

I observed that, The XML content is generating properly.
But while transforming from XML to HTML code, the url's are not constructing
properly.

Please help me in fixing the issue by telling in which direction I need to
work.
