[gradebook2-dev] Instructors can grade students on deleted grade items

As Thomas said, we have identified a number of issues that seem to
come from the same cause as this one.... namely the summary panel is
saved after it is opened the first time.... so the state needs to be
managed in the (often not visible) panel.... unfortunately, it's not
being managed all that well... we've favor the proposal to create the
student panel anew each time it is accessed, since that is the easiest
way to manage the state while it is inactive: don't :)

If anyone has any comments or concerns about that approach, plz bring them up
