[Contrib: Evaluation System] EvalSys-1063 -viewing results

Version 1.4.1 of evaluations is running on the http://qa5-us.sakaiproject.org/portal server -- this shows the behavior of the checkbox always checked but also always grayed out. 

It looks like a pretty easy change to both remove the checkbox and have it default to unchecked. Do you have developers that can work on this? I'd suggest the following:

1) Select the "Subversion" link from https://jira.sakaiproject.org/browse/EVALSYS-1063 to see the changes made to implement this
2) Modify the EvalAssignHierarchy.java and evaluation_settings.html files to set the value to false and remove the checkbox

We'll try to do some testing to see if the UI is misleading or if the "instructorsViewAllResults" is indeed always true, but are interested in if you are able to find more out as well.
