[Contrib: Evaluation System] Survey error

We had a situation where a survey creator got an error page just after pressing finish on the 'assign participants' page (Evals 1.3). this was done in a classroom situation with a handful of people creating surveys from templates at the same time.

Any hints gratefully accepted.

The survey was actually created OK but we saw the following in the logs. Has anybody got any ideas?
