[Contrib: Evaluation System] 3 issues for evals in v1.3

Does anyone have any more ideas on this? It would be nice if we could figure this out before the conference, but if not, hopefully Cathy could walk through her workflow with one of our esteemed Evaluations power users :-)

The only error I see in the logs is the following -- don't know if it's relevant, but it's generated when Cathy reproduces the error case mentioned below (she can't create an evaluation from a template).
