[Contrib: Evaluation System] Evalsys update from UCT

Hi all,

UCT recently put an updated local branch into production, which is approximately trunk plus a set of new features (not yet in trunk).

The umbrella issue for this is:

Linked issues that are resolved are in trunk; unresolved issues are implemented in our branch but not in trunk.

We have done a reasonable amount of QA for the features exposed by UCT's use cases (which excludes hierarchy but includes the "self-service" life-cycle including template editing, etc.) and have a test plan which I will dig out and post to the list or place on Confluence soon.

On the plus side, we've successfully moved to much more recent evals code and tested and fixed a number of issues in trunk that will be of immediate benefit to others. On the minus side, we're still ahead of trunk whereas where we'd ideally like to be is behind trunk using a maintenance branch, but perhaps we'll get there for our next update (which could be to a 1-4-x branch or something).

In due course we need to schedule a teleconference or some discussion on the outstanding issues that are candidates for merging to trunk. I don't think any of these should be disruptive or controversial, but if you see something in an issue linked to EVALSYS-765 which you think would be problematic for your institution, please add a comment to the relevant issue.
