Alignment is way off on my Mac viewing the website using FF3.04 on Mac OS 10.6.2.  With that said, I like what you've done in terms of the hovering over a choice and seeing a header get highlighted.  This, of course, only works well when you can actually see the item labels in your survey.  If you have a long block of items the labels can be off the screen.

As for the NA option, we are using the horizontal format now and have been since the start so our NA is at the front before the actual answers.  We would want the NA to REMAIN where it is now for consistency.  We do not use the vertical display format.
